Insurance Information

Our insurance coordinators deal with many different insurance companies. Some companies offer many different dental and medical plans. Some companies change benefits, co-pays and deductibles faster than we (or you) can keep up with. We do our best to provide you with accurate coverage estimates based on information available to us. At times, it is almost impossible to accurately estimate our patients’ insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can be difficult and time consuming. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance is current. 

As a courtesy to you, we will gladly file your claim on your behalf in a prompt manner. If necessary, we will file your claim/predetermination of benefit a second time. If your claim is denied, it will be your responsibility to follow up with your insurance company to resolve the claim. Keep in mind that Dental Insurance is a contract between you, the patient, and the insurance company. You are responsible for your own account, and not all services are a covered benefit in all contracts.

Insurance companies contract with your employer to pay certain percents for dental services. The more your employer pays for a plan the better the coverage. Often, the term “Usual”, “Reasonable”, or “Customary” is used by the insurance company. This differs between all companies and there is a maximum at which the insurance company will pay for a service. It in no way dictates your dental fee, it is simply the maximum your insurance company will pay for a particular service according to the plan your employer has purchased. For example:  80% coverage means 80% of the insurance company’s maximum for your plan – not necessarily 80% of our dental fee.   Your insurance has a yearly maximum and it is your responsibility to keep track of remaining funds.

Dr. Cummings will propose a custom treatment plan for your specific case and recommend appointments with your health in mind, not your insurance coverage.

Private & Group Insurance “Flex Plans”

As a courtesy to our patients with medical and/or dental benefit plans, we will submit necessary claim forms, receipts, and other information to your insurance company. Upon receipt of an insurance payment, any balance due will be the responsibility of the insured and we will bill you accordingly. If you have deposited an excessive co-payment, it will be refunded to you.
